I, {name}, fully and voluntarily consent to participation of myself and/or my child in programs, classes, and seminars held by 2:22 Ministries Inc. (DBA Pure Krav Maga). 

I hereby acknowledge my awareness that participation may expose me/my child to risk of property damage, bodily or personal injury, mental and physical harm or death including but not limited risks from sparring, lifting, full contact striking, falls, and crossing streets, parking lots and intersections. I understand that the risks that I/my child may encounter include, but are not limited to injury from falls, bumps, bruises, cuts and abrasions, muscle strains and sprains, fractured or broken bones including teeth, injury to the eyes, and exposure to contagious diseases which may cause death, as well as other risks that may not be foreseeable. I knowingly and freely assume any and all such risks. I understand I should carry my own health, dental and eye insurance in the case of injury. 

In exchange for being allowed to participate in the Program, I hereby release and forever discharge and agree to indemnify 2:22 Ministries (DBA Pure Krav Maga) its members individually and their officers, agents, volunteers and employees from any and all claims, demands, rights, expenses, actions, and causes of action, of whatever kind, arising from or by reason of any personal injury, bodily injury, property damage, or the consequences thereof, whether foreseeable or not, resulting from or in any way connected with my participation in the Program. 

I further covenant and agree that for the consideration stated above, I will hold forever harmless and will not take legal action against 2:22 Ministries, its members individually, and their officers, agents, and employees for any claim for damages arising or growing out of my participation in this activity whether caused by negligence or otherwise. I certify that I understand and have read the above carefully before signing. I acknowledge and represent that I freely and voluntarily sign this Agreement, and that it is my express intent that this Agreement shall contractually bind my heirs, executors, administrators, and assigns, and my child’s heirs, executors, administrators, and assigns, as well as myself and my child.

PHOTO RELEASE

I hereby give permission to Pure Krav Maga, LLC and its employees, representatives, or associates permission to photograph, print, and record of myself, family including minors, and guests for use in electronic or printed publications, media (including social media), and advertising. This permission is given with the understanding that no monetary compensation will be received for participation in the photograph, sound, or video media. I further release Pure Krav Maga, LLC, its employees, representatives, associates and the photographer (professional or otherwise) from any and all liability, known and unknown, arising out of the use of this material. 

This Agreement sets out your rights to use our Facilities and Services and the responsibilities you have to comply with as a Member. These responsibilities, including payment of Membership Fees, do not depend on how often you use our Facilities and Services. What is set out in this Agreement overrides any statements made by you or us before you signed it so you should read through it fully to make sure it reflects your expectations. If you are unsure whether any particular statements that you have relied on are part of this Agreement, or if you have any queries regarding this Agreement, please ask us before you sign.

Direct Debit: This Agreement allows for the Membership Fees to continue to be debited from your account until you or we cancel the arrangement by advising your bank or credit provider. If you end this Agreement or stop the automatic debit arrangement in a way not described in this Agreement, you may be liable for damages for breach of contract. Any changes to payments, fees, and/or dues in the case of added members or transfers to another member, is done without the need of a new agreement and terms in this agreement apply to any and all memberships therein. If you wish to re-sign a new agreement, you may request one. Credit and Debt cards used agree to pay any card processing fees.  ACH: Cash or Direct bank transfers receive a 3.99% discount.

Your Safety: You agree to give us all relevant health and fitness information before or during any exercise. Each time you use the Facilities and Services, you must ensure you are in good physical condition and know of no medical or other reason why you should not exercise. If unsure, you should seek medical guidance. You must sign a waiver for yourself and on behalf of any minors on your membership. Failure to do so will terminate your membership immediately. 

Holds: You may freeze or "hold" you agreement for up to 3 months. A minimum hold period of one payment cycle can be applied. Your membership will be extended the length of the freeze period. There are no fees applied for holds. Memberships will resume automatically without notice at the end of the hold period. Any hold must be in writing and may be sent in email to info@gakravmaga.com. 

Cancellation: You may cancel at any time, for any reason, with a 30 day cancellation notice. You will still be allowed access to training for the remainder of that time. There are no fees to cancel but any payment within the 30 day cancellation period will be assigned. Your cancellation must be submitted in writing and may be sent in email to info@gakravmaga.com. You may be asked to complete an exit questionnaire. Cancellation will not be processed until confirmation of the completed exit questionnaire is received, acknowledged and confirmed. By entering this agreement you understand that if you do not answer the exit questionnaire within the 30 day cancellation period, your cancellation will not be confirmed and you will continue to be charged at the agreement rate. If you do not receive an exit questionnaire within the 30 day cancellation period, your cancellation will automatically be granted. 

Refund Policy: Any refund is at the discretion of The Board of 2:22 Ministries Inc. Refund requests must be submitted in writing to info@gakravmaga.com. Refunds for defective merchandise will only be given if an exchange is not possible. Refunds for memberships will only be given in the case of billing errors. Cancellation of membership does not constitute a right to refund on any unused time remaining on a membership. In the case a refund is granted, your refund will reflect the subtraction of any additional fees incurred through processors or banks.  

Transfer: This Agreement is not transferrable to anyone outside of your household. You may assign your membership to another person in your household at any time and they will be subject to all of the same terms herein. 

Fees: You will be charged any fees imposed by the bank or processor for any late, returned, failed or stopped payments and will be charged an additional $10 fee. 

Delinquent Membership: Failure to pay any membership payment or fees will result in loss of access to class unless other arrangements have been made. Access to class does not equal cancellation and future payments will accrue against the membership. In the case of arbitration or litigation, any fees and/or expenses resulting from delinquent payments will be pursued to the full extent of applicable laws. 

RULES 

All members and their guests will be required to follow safety guidelines, rules and restrictions. Any failure to do so will result in a termination or administrative cancellation to your membership. 2:22 Ministries operating as Pure Krav Maga retains the right to terminate or administratively cancel your membership at any time. At the time of termination, the final payment will be taken immediately and you forfeit the right to the remainder of your membership without a cancellation period. 

1. Be kind and courteous of members, guests, other students, and instructors. Harassment, battery and/or assault in and out of the gym will not be tolerated. Verbal threats, physical harm, and mental trauma or manipulation is grounds for immediate forfeiture of membership. Do not use or share personal information or contact information without consent. 
2. Personal hygiene practices should be maintained. Be mindful of sweat, odors, and germs when training closely with other members. Wear appropriate and clean clothing. Trim and file your nails to avoid scratches. 
3. No spitting on the mats and wash your hands before returning to the mats and after mouthguard removal. 
4. Shoes are optional in class. Any shoes that are worn during class will be clean and free of any debris or dirt. If shoes are not being worn, feet must be clean and nails trimmed. 
5. No jewelry, metal accessories or clothing attachments that can cause harm including but not limited to cuts, bruising, abrasions, and/or strangulation. 
6. Do not come to class with any transmissible illness including but not limited to rashes, infections, intestinal upset and/or fever. You may return to class once you have been free of symptoms for 48 hours. 
7. Any gear that is loaned will be returned clean and in working conditions. Parents of children will enforce cleaning of any messes at any time on the premises. Misuse and harsh treatment of any equipment, pads, or gear will be charged to the responsible member or parent for the price of a replacement. 
8. Recording by a member in class is for personal use only. Do not post content on social media of minors or any other members without permission. Any content from classes, seminars, training or online member content is the property of 2:22 Ministries/Pure Krav Maga. You agree not to share any member exclusive content with anyone who is not a current member. 
9. Follow Instructions! Be aware that by ignoring instructions you increase the risk of injury to yourself and others around you. 
10. No Bullying. Parents agree that in the event that their child uses tactics against anyone when they are not at risk, they are to inform an instructor. This includes school and activities inside and outside the gym. Children will enter into a No Bullying Contract to be signed by the child and the parent and a corrective disciplinary period will be listed in that contract. No holds will be granted during the disciplinary period. If the disciplinary period is not completed as outlined in the no Bully Contract, the membership may be terminated. If bullying continues or is repeated, the membership will be terminated. 

PHOTO RELEASE

Upon entering this agreement, you acknowledge the absolute and irrevocable right and unrestricted permission to publish, distribute, exhibit or otherwise use the photographs and/or video taken of any members or their guests.  This includes the release from all claims, demands and liabilities whatsoever in connection with the photographs and videos.
Images may be modified in any manner, and any right to inspect or approve any final product is waived by the member.
The images may be used, without limitations, in print publications, online publications, websites and on social media.
Any right to royalties or other compensation arising from or related to the use of the photographs and/or videos is also waived.
